Appointments
If you would like to make an appointment, please contact our office by phone or email. Our qualified office staff will help you with any questions you might have. Office hours are Monday to Friday from 8am-5pm.
Financial
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. If you would rather submit insurance forms yourself, we will provide you with the necessary paperwork to do so. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted are check, cash, debit, and most major credit cards.
Cancellation
If for some reason you are unable to keep a scheduled appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advanced notice so that we can offer this appointment to another patient who may be in need of treatment and not be able to get in otherwise.
